Hours
Cascade Summit Physical Therapy is open Monday through Friday from
8:00am to 6:00pm at both locations.
Appointments
To make an appointment, call the respective clinic. Regular attendance
at your scheduled appointment is essential for you to get the maximal
gain out of your rehabilitation. If you are unable to keep your scheduled
appointment time, please have the courtesy to let us know so that we
may use that appointment slot for another client, if needed.
First Appointment: Please arrive 15 minutes early
to allow time for filling out paperwork. Bring your insurance card,
referral form (if needed by your managed care company), and the Physical
Therapy Prescription from your doctor. It is also helpful to wear comfortable
exercise clothing that will allow your therapist to easily examine your
injured area, will allow you freedom of movement in initiating your
rehabilitation program. Your first appointment usually takes about an
hour and a half and there is minimal waiting time. We will also keep
your referring physician informed regarding your status with periodic
progress reports.
Referrals: Many insurance plans require a written
referral from your primary care physician. This is different from a
Physical Therapy Prescription. If a referral is required by your plan,
it must be presented at the time of your first visit. Most patients
find it helpful to call their insurance company before the first appointment
to determine their level of coverage and co-pay information. Our office
staff is always available to assist you if you have any questions concerning
your insurance coverage.
Fees: Our fees are the standard reasonable and customary
charges found in the Central/Eastern Washington region. If you have
any questions about the fee for a specific visit, please do not hesitate
to ask a staff member for clarification.
Insurance
Many insurance plans require deductibles and/or co-payments at the
time of the visit; you may pay these with cash, check, VISA or MasterCard.
If there is an insurance company with a rehabilitation benefit, we have
a contract with them. These include, but are not limited to:
- Regence
- Premera
- Medi-care
- Medi-caid and derivatives
- Group Health
- State Industrial
- Tri-care (Military)
- and many other plans
Private Pay Policy
Payment for evaluation fee is due at time of visit, $50.00 is due at
each follow up appointment.
